Golf Outing Registration
 
You may register the following ways:
    1. Print this completed form, submit online, and fax to NASRO Member Services, 1-651-789-0445. Attn: Barry Orton/NASRO
    2. Print this completed form, submit online, and mail to NASRO, 1951 Woodlane Dr., St. Paul, MN 55125. Attn: Barry Orton

    Checks, money order (payable to NASRO) may be included within mailed registration.

    In addition, NASRO accepts VISA/MASTERCARD. If you choose this method of payment, please note the 16 digit card number, expiration date, cardholder name and card billing address.

  • July 30th, 2008 with a 1:00 pm "Shotgun" start.
  • JW Marriott's Nick Faldo Championship Course.
  • $65.00 (inclusive) per person if registered and paid prior to conference, or $75 if paid on site (We will attempt to honor all foursome requests). Fee includes green fees, mandatory golf cart, practice balls, and box lunch.
  • Beverages will be available for additional purchase during the outing.
  • Rental Clubs are available for an additional $40.00. Rental shoes available for $20.
  • Collared shirt required. No denim or jeans allowed.
    Cancellation Policy: Enclosing payment with the pre-registration form guarantees you a golf outing slot. If you need to cancel, please note the reimbursement policy highlighted below:
  • Full refund given if written notification is received by NASRO by June 30, 2008.
  • 50% refund given if written notification if received by NASRO July 1-26, 2008.
  • No Refunds after July 26, 2008.
