    2. Print this form and mail to NASRO, 1951 Woodlane Dr., St. Paul, MN 55125. Checks, money order (payable to NASRO) may be included within mailed registration. In addition, NASRO accepts VISA/MASTERCARD. If you choose this method of payment, please note the 16 digit card number, expiration date, cardholder name and card billing address.

   ** Late fee of $50.00 per registrant will be assessed if registration fee received after July 21, 2008.

   Refund Policy: Refunds will be permitted if NASRO is notified in writing prior to June 21, 2008.  Requests 
    for refund after June 21, 2008 must be of an emergency nature and in writing to the Board of Directors 
    from agency head.  Any refunds processed after June 21, 2008 will be subject to a $100 processing fee.  
    Any requests after the conference must be received no later than August 26, 2008.
