notified in writing prior to June 21, 2008. Requests
for refund after June 21, 2008 must be of an emergency nature and in writing to the Board of Directors
from agency head. Any refunds processed after June 21, 2008 will be subject to a $100 processing fee.
Any requests after the conference must be received no later than August 26, 2008.
